Attached is the Course Packet Information for your BCM6-16-11 Creating a Mathematics Progress Monitoring System for Special Education - BURLINGTON class you are teaching.
Included in this are Instructions, contract(if needed), and Online Evaluation Instructions. Please print this information off and follow the directions below regarding printing sign-in sheets, payments etc.
Also attached is a Registration Form for Viterbo, you will only need this if your course is being offered for credit through Viterbo. If so please print the form and have it available for students in your course who have selected this type of credit. They need to return the form to Julie Hannam in our office, not the Viterbo Office.
We are using the MyLearning Plan program to simplify the instructor’s materials.

1. You need to print a current sign-in sheet for each meeting date.
2. Please have students sign in and initial for the credit they registered for. If they would like to change the type of credit, make necessary changes on this sheet. They are only allowed to change on the first day of class. No one is allowed to change a credit type after the first meeting day. If someone is not on the sign-in sheet they are not allowed to attend, pre-registration is required. If someone is on the list and did not show up please make sure you write on the signature line one of these 3 options.
  • No Show – if they never showed up
  • Dropped – if they called prior to course start date
  • Absent – if they have made prior arrangements with instructor
3. On your sign-in sheet is a payment status, it is located at the bottom of their name, address, etc. It lists the amount that is owed for each person. Make all checks payable to GPAEA and mail or route to Leslie Roberts in the Burlington Office. Instructors are not to accept payments, they are to be mailed to Leslie Roberts prior to the class start date. If students bring a check to class please instruct them to mail it to Leslie.
4. At the end of your course: Record your grades in the Submit Activity Attendance function, hit save and send an e-mail to Julie Hannam letting her know your grades have been submitted. Turn in your sign-in sheets and contract to Julie Hannam.
5. Instructor’s Contracted Payment will be sent after the next board meeting.
